Licensed Practical Nurse
To participate in the planning and delivery of patient care services under either minimal or direct supervision of an RN and directs patient care activities in the absence of an RN. The LPN may function as a Shift Charge Nurse, directing the delivery of patient care on the Skilled, Intermediate, and Residential Care Units for an assigned shift.
Job Description
- Functions within the scope of practice for a Licensed Practical Nurse in the State of Oregon.
- Maintains the confidential nature of the work environment.
- Participates in the planning and organization of patient care.
- Coordinates with other departments and care providers the delivery of patient care services.
- Performs detailed assignments, works under pressure, and relates positively to patients, staff, and visitors.
- Is familiar with the current condition of each patient.
- Routinely monitors resident care to observe patient condition and monitor delivery of care.
- Participates in the establishment of the care plan.
- Initiates emergency measures as necessary.
- Reports changes in patient condition to Physician, family, and as appropriate, to other caregivers.
- Maintains complete and accurate patient records, providing timely, accurate, current documentation.
- Maintains a safe, clean, and comfortable environment for patients.
- Maintains and updates own clinical skills and learns new skills as necessary to perform unfamiliar assignments.
- When functioning as Shift Charge Nurse, adjusts daily Nursing Department staffing in both the current shift as well as upcoming shifts by calling in or dropping personnel as needed.
- Assures timely acquisition of patient observations made by other staff members.
- Assures adherence to new and established policies and procedures by instruction, supervision, and example.
- Collaborates with the DNS in formal performance evaluations.
- Able and willing to work beyond normal working hours and in other positions temporarily if necessary.
- When functioning as Shift Charge Nurse, initiates employee counseling and/or disciplinary action as indicated by infraction.
The Charge Nurse may:
- Administer first formal warning for policy infractions.
- Administer suspension from work of current shift.
Administers IV Therapy according to established policies and procedures if certified in IV Therapy. Maintains harmonious shift/inter-shift and departmental/interdepartmental working relationships. Represents the interests and concerns of co-workers to Shift Charge Nurse or DNS. Provides leadership and positive direction in accordance with, and in support of, established philosophy, objectives, policies, and procedures.
Daily:
- Does dressing changes, treatments, ostomy care, catheter care, catheter changes assigned.
- Gives all G-tube medications during hours assigned.
- Completes all accuchecks and insulin injections during work hours.
- Records all vital signs taken during the night.
- Completes other tasks assigned by the charge nurse or RNCM.
Requirements
- Current valid Oregon License as a Licensed Practical Nurse.
- Current CPR card.

Must function independently, have flexibility, and the ability to work effectively with residents, personnel, and support agencies and to maintain a calm and harmonious work environment.
