Job Description:
- Provides services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries or disease
- Examines patient's medical history, evaluates their physical abilities, determines their potential to respond to therapy, and develops treatment plans
- Provides direct patient care, evaluates functional needs and outcomes, and establishes plan of care
- Conducts therapy practice within defined standards of care
- Evaluates effectiveness of treatment and modifies as necessary
- Assesses educational and cultural needs of the patient, family, caregiver, or significant other and communicates accordingly to achieve maximum benefit of care
- Documents evaluations, treatment goals, and plans in accordance with Hospital requirements and established guidelines
- Regularly updates documentation and maintains prescriptions and signatures
- Prepares plans for discharge
- Coordinates, delegates, and supervises responsibilities assigned to support staff
- Coordinates with interdisciplinary team on patient needs, treatment modifications, and progress
- Completes and files patient documentation in a timely manner
- Attends and participates in the weekly team conference
- May be required to work during inclement weather and other staffing emergencies
- Provides an environment conducive to safety for patients, visitors, and staff
- Assesses the risks for safety and implements appropriate precautions
- Complies with appropriate and approved safety and Infection Prevention standards
- Maintains regular attendance in keeping with assigned work schedule
- Performs other duties as assigned to support overall effectiveness of the organization
Requirements:
- Two years direct patient physical therapy care and treatment experience preferred
- Experience in rehabilitation hospital setting preferred
- Current state licensure as a Physical Therapist required
- Basic Life Support Certification required
Skills and Abilities:
- Demonstrates appropriate application of age-specific standards, policies, procedures, and guidelines in caring for pediatric, adolescent, adult, and geriatric patients
- Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS
- Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients
- Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency
- Demonstrates general computer skills including data entry, word processing, email, and records management
- Demonstrates critical thinking skills
- Effective organizational and time management skills
- Effective written and verbal communication skills
- Ability to prioritize, meet deadlines, and complete complex tasks
- Ability to maintain quality, safety, and/or infection prevention standards
- Ability to work independently
- Ability to maintain proper levels of confidentiality
- Ability to work closely and professionally with others at all levels of the organization
Physical Requirements:
- A significant amount of standing, walking, bending, squatting, reaching, lifting, pushing, and pulling, often for prolonged periods of time
- Both gross and precise motor functions
- Lifting/exerting of up to 50 lbs
- Possible exposure to bodily fluids
- Visual acuity required for patient assessment and documentation of care
- Acute hearing required for accurate patient assessment
- Sufficient manual dexterity to operate equipment and computer keyboard
