    Job Description

    • Provides direct patient care, evaluates functional needs and outcomes, and establishes plan of care; also evaluates the efficacy of treatment, modifying if necessary. Conducts all therapy practice within defined standards of care.
    • Documents evaluations, treatment goals, and plans in accordance with hospital requirements and established guidelines. Regularly updates documentation and maintains prescriptions and signature in a timely manner.
    • Coordinates, delegates, and supervises responsibilities assigned to support staff.
    • Coordinates with interdisciplinary team on patient needs, treatment modifications, and progress including preparing plan for discharge.
    • Provides an environment conducive to safety for patients, visitors, and staff. Assesses the risks for safety and implements appropriate precautions. Complies with appropriate and approved safety and infection prevention standards.
    • May be required to work during inclement weather and other staffing emergencies.

    Requirements

    • A minimum of 2 years' experience in inpatient hospital setting; inpatient rehabilitation experience preferred.
    • Current state license as a Physical Therapist in good standing is required.
    • Current BLS/CPR certification required.
