Registered Nurse - Home Care
As a Registered Nurse in home care, you will be responsible for assessing patients' health needs, developing personalized care plans, and delivering high-quality healthcare services in the patients' homes. You will work closely with patients and their families to ensure their physical, emotional, and psychological well-being while promoting independence and improving quality of life.
Job Description
- Conduct initial assessments of patients' health status and needs.
- Develop comprehensive care plans tailored to each patient's specific requirements and goals.
- Monitor and evaluate patients' progress regularly, adjusting care plans as needed.
- Provide skilled nursing care such as medication management.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated care.
- Document patient care activities accurately and timely in accordance with regulatory requirements and agency policies.
- Provide emotional support and counseling to patients and their families, addressing their concerns and facilitating coping mechanisms.
- Ensure compliance with safety protocols and infection control measures to maintain a safe environment for patients and caregivers.
- Participate in ongoing training and professional development activities to stay abreast of advances in nursing practice and home care standards.
- Advocate for patients' rights and promote their autonomy and dignity in decision-making processes.
Requirements
- Valid RN license in the state of Virginia.
- Minimum of 2 years of nursing experience, preferably in a home care or community health setting.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Proficiency in using electronic health record (EHR) systems and other relevant healthcare technologies.
- Reliable transportation and willingness to travel to patients' homes.
- CPR certification (may be required).
- Bachelor of Science in Nursing (BSN) preferred.
Working Conditions
- Flexible scheduling, including evenings and weekends, to accommodate patients' needs.
- Exposure to various home environments, which may include limited space, pets, and other household factors.
- Physical demands such as lifting and transferring patients, bending, and prolonged standing.

- $38.00 - $40.00 per hour
