    Registered Nurse - Home Care

    As a Registered Nurse in home care, you will be responsible for assessing patients' health needs, developing personalized care plans, and delivering high-quality healthcare services in the patients' homes. You will work closely with patients and their families to ensure their physical, emotional, and psychological well-being while promoting independence and improving quality of life.

    Job Description

    • Conduct initial assessments of patients' health status and needs.
    • Develop comprehensive care plans tailored to each patient's specific requirements and goals.
    • Monitor and evaluate patients' progress regularly, adjusting care plans as needed.
    • Provide skilled nursing care such as medication management.
    • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated care.
    • Document patient care activities accurately and timely in accordance with regulatory requirements and agency policies.
    • Provide emotional support and counseling to patients and their families, addressing their concerns and facilitating coping mechanisms.
    • Ensure compliance with safety protocols and infection control measures to maintain a safe environment for patients and caregivers.
    • Participate in ongoing training and professional development activities to stay abreast of advances in nursing practice and home care standards.
    • Advocate for patients' rights and promote their autonomy and dignity in decision-making processes.

    Requirements

    • Valid RN license in the state of Virginia.
    • Minimum of 2 years of nursing experience, preferably in a home care or community health setting.
    • Excellent clinical assessment and critical thinking skills.
    • Strong communication and interpersonal skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Compassionate and patient-centered approach to care.
    • Proficiency in using electronic health record (EHR) systems and other relevant healthcare technologies.
    • Reliable transportation and willingness to travel to patients' homes.
    • CPR certification (may be required).
    • Bachelor of Science in Nursing (BSN) preferred.

    Working Conditions

    • Flexible scheduling, including evenings and weekends, to accommodate patients' needs.
    • Exposure to various home environments, which may include limited space, pets, and other household factors.
    • Physical demands such as lifting and transferring patients, bending, and prolonged standing.

    • $38.00 - $40.00 per hour
