Transitional Care Nurse Practitioner
Dignity Health Medical Group - Arizona is seeking a Transitional Care Nurse Practitioner to join our team in Phoenix, AZ. In collaboration with the physician/medical director, the nurse practitioner assumes clinical responsibility for tasks performed within their scope of practice. This includes taking patient histories, performing physical examinations, ordering appropriate diagnostic tests, establishing diagnoses, and providing care and management for common illnesses and disease prevention in pediatric and adult patients. The Advanced Practice Provider (APP) may assist in the admission, transfer, or discharge from the healthcare system, make appropriate referrals to specialists and community agencies, and participate in quality assurance and quality improvement programs. The APP works within a multidisciplinary team to develop plans for patients and their families.
Job Description
- Engage in direct patient care activities and medical/clinical practice as the principal component of overall professional responsibility.
- Conduct assessment, evaluation, diagnostic determination, ongoing treatment and clinical management, care planning and modification, medication reconciliation, risk assessment, and coordination of care with Acute Care Providers, Transitional Team, and Primary Care Physicians.
- Maintain direct contact and consultation with patient family members, advocates, and others to ensure effective communication of patient status and care plans.
- Manage day-to-day patient care activities that include professional input from all members of the patient care team.
- Complete all clinical and professional records and documentation accurately and timely to assure coordination of care.
- Perform other related duties incidental to the work described herein.
Requirements
- NP Master's Degree from an accredited program.
- Three years of clinical experience in healthcare.
- AZ license; registered in AZ as an NP.
- Hold and maintain national certification in NP area under AZ regulations.
- American Heart Association BCLS certification.
Preferred Qualifications
- 1+ years of NP experience.
About the Position
A Transitional Care Nurse Practitioner for Dignity Health Medical Group is an Advanced Practice Provider who provides clinical care for high-risk/complex care patients recently discharged from a Dignity Health facility. The goal of the APP is to prevent unnecessary hospital readmissions by providing improved coordination of care and convenient post-acute care access to medical care. Patients will be seen in clinic Monday - Friday from 8 AM to 5 PM. Additionally, all providers within this clinic rotate in on-call requirements. The Department of Transitional Care currently consists of five APPs and one physician along with other resource staff across two clinics: St. Joseph's Hospital and Medical Center and Chandler Regional Medical Center.
Pay Range
$51.66 - $82.14/hour
