    Community Health Physician - Geriatric Service Line

    Job Description

    • Lead a novel geriatric service line with a dual function to provide direct patient care and consultative/educational care for the local provider community.
    • Design programs, provide clinical care, and oversee clinical operations.
    • Establish and nurture relationships with internal and external stakeholders.
    • Serve a consultative and supportive role to the independent Skilled Nursing Facility (SNF) service line.
    • Work at a newly constructed facility in Rutland, Vermont.

    Requirements

    • Graduate from an accredited medical school and completed FM/IM residency.
    • Board certification in Internal Medicine or Family Medicine.
    • Geriatric fellowship and subsequent Board certification recommended (not required).
    • Must be eligible for licensing by the State of Vermont.
    • Related experience working with the geriatric population.
    • Effective communication skills.
