Community Health Physician - Geriatric Service Line

Job Description
- Lead a novel geriatric service line with a dual function to provide direct patient care and consultative/educational care for the local provider community.
- Design programs, provide clinical care, and oversee clinical operations.
- Establish and nurture relationships with internal and external stakeholders.
- Serve a consultative and supportive role to the independent Skilled Nursing Facility (SNF) service line.
- Work at a newly constructed facility in Rutland, Vermont.
Requirements
- Graduate from an accredited medical school and completed FM/IM residency.
- Board certification in Internal Medicine or Family Medicine.
- Geriatric fellowship and subsequent Board certification recommended (not required).
- Must be eligible for licensing by the State of Vermont.
- Related experience working with the geriatric population.
- Effective communication skills.
