Occupational Therapist Home Health
Join our team as an Occupational Therapist in Home Health, serving the special purpose of caring for Valley patients in the safest place to them, their home. In this role, you will provide occupational therapy services to patients, focusing on assessment, treatment, program planning, and implementation to help patients achieve optimal functional independence.
Job Description
- Provide occupational therapy services to patients in their homes.
- Collaborate with a multifaceted team of seasoned therapists and mid-career clinicians.
- Average 25-30 visits per week and establish individualized plans of care.
- Work within an interdisciplinary team caring for patients with conditions such as Stroke, Traumatic Brain Injury, Spinal Cord Injury, Cardiac, Orthopedic, and more.
- Prioritize teamwork, mentorship, and create a teaching environment.
Requirements
- Bachelor's degree in Occupational Therapy.
- Completion of internship program in Occupational Therapy required.
- State of California Occupational Therapy license or eligible for licensure.
- Valid California Driver's License and driving record acceptable to Community Health System required.
- Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required.
