Job Description

• Provide care tailored to the physical, cognitive, and psychosocial needs of adult and geriatric residents.
• Administer medications and treatments accurately, monitoring for effectiveness and documenting in accordance with policies.
• Deliver nursing care to residents with therapeutic, neuro, dementia, dialysis, IV therapy, infectious disease, and wound care needs, as applicable.
• Ensure compliance with state codes and facility protocols to maintain the safety and well-being of all residents.
• Collaborate with the interdisciplinary team to follow and implement individualized Care Plans.
• Perform additional duties as assigned to support resident health and facility operations.

Requirements

• Graduate of an accredited nursing program.
• Active LPN license in the state of Pennsylvania, in good standing.
• Strong communication, organizational, and problem-solving skills.
