Palliative Care / Geriatric Medicine Physician

Job Description:

• Join a well-established program with both outpatient and inpatient palliative care medicine
• Work alongside two seasoned Palliative Care APPs focused on inpatient care
• Office includes geriatric and transitional care medicine, complementing the palliative care program
• Epic EMR system in place
• Vibrant inpatient program
• Outpatient practice located in the same department as Geriatrics Practice
• Call rotation includes inpatient weekend coverage
• Schedule: Monday - Friday (8am - 5pm)
• Primary collaborative physician for two Palliative Care Inpatient NPs and a transitional care NP in the outpatient clinic
• Alternative collaborative physician for two NPs focusing on geriatric medicine within the outpatient clinic
• Access to EKG Machine, Sophia-Flu, Covid Reader, and A1C testing
