Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri!

Job Description:

• Utilize evidence-based practice to support clinical interventions
• Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist
• Provide skilled therapy services to patients who need rehabilitation services
• Assess and document patient's progress and communicate with supervising Physical Therapist to adjust treatments accordingly
• Provide consultation and counseling to patients, as well as families, caregivers, and other service providers related to physical disorders
• Provide input to supervisor for discharge summary of services provided per regulatory and clinical requirements

Experience/Qualifications:

• Licensed Physical Therapy Assistant
• This position is open to therapists at all experience levels
