Clinical Pharmacist - Per Diem
Location: Lebanon, NH
Job Description:
- Engage patients and healthcare professionals in an outpatient setting to maximize therapeutic outcomes.
- Responsible for tasks including dispensing medications, monitoring drug therapies, providing consultations, and educating patients and staff.
- Work within a high Pharmacist:Technician ratio team.
- Position includes an overnight shift with a 20% shift differential, and rotating day/evening shift per diem opportunities.
Responsibilities:
- Gather complete medication histories and reconcile medication lists.
- Conduct comprehensive medication reviews for evidence-based recommendations.
- Identify and document adverse drug reactions and adherence gaps.
- Consult with providers on medication safety and appropriateness.
- Serve as a consultation resource and answer drug therapy inquiries.
- Assist in discharge planning and educate patients on medication therapy.
- Contribute to the development of medication guidelines and detect/report medication errors.
- Prepare and deliver educational presentations.
- Participate in quality improvement and medication use review activities.
- Maintain pharmacy system efficiency and supervise technicians.
- Operate as pharmacist-in-charge in absence of managers.
Qualifications:
- Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy. Doctor of Pharmacy (Pharm D) degree preferred.
- Completion of a post-graduate residency and/or board certified preferred.
- Strong written and verbal communication skills.
- Licensed to practice in the state of NH, with willingness for license reciprocity if required.
Required Licensure/Certifications:
- Current, unrestricted pharmacist license to practice in NH.
- BLS Certification within 30 days of hire for Clinical Pharmacists in Retail or Specialty Pharmacy.
- Board Certification in Specialty Pharmacy (CSP) within 3 years of hire for Clinical Pharmacists in the Specialty Pharmacy.
Professional Development:
