    Sr. Program Manager - HOST

    Job Description

    • Oversee the operations and administration of the HOST Mental Health and HOST Substance Use Disorder programs and their components.
    • Ensure timely upkeep of the HOST operations manual, including program overview and standards, staffing patterns, outreach and engagement best practices, program reporting standards, documentation best practices, and relevant forms and templates.
    • Oversee formal HOST training curriculums that include program vision, assertive outreach and engagement best practices, building community partnerships, roles in a multidisciplinary team, documentation standards, and navigating scenarios in the field.
    • Participate in public engagement and promotion of the HOST program citywide.
    • Work in-person at several different sites during the work week.
    • Collaborate effectively with other agencies in the community working with unhoused individuals, bringing DESC expertise to help influence and improve outreach systems.
    • Help build and maintain relationships with BIPOC and other agencies that specialize in diverse client populations, including LGBTQ, disability, religious, and other communities.
    • Directly supervise the HOST Project Manager.
    • Participate in team meetings with supervisors and staff.
    • Help develop and implement clinical onboarding and ongoing training for the project manager, supervisors, and direct line staff.
    • Help negotiate contracts in coordination with the clinical contract manager.
    • Help build and refine reports needed to illustrate program outcomes.
    • Work in coordination with the Quality Improvement Management department to ensure all auditing, quarterly reviews, and documentation guidelines are met.
    • Establish and maintain relationships with program monitors at the city, county, and state levels.
    • Serve as part of a weekly on-call Project Manager rotation to provide real-time assistance either remotely or in person throughout MRD division programs.

    Street Medicine Team

    • Establish and maintain relationships with program partners including Public Health and Evergreen Treatment Services.
    • Collaborate and adjust program operations to meet the unique needs of the community based on patterns related to medical, behavioral health, and substance use.
    • Participate in identifying key stakeholders and building relationships to reach a wide range of individuals across King County.

    Program Management & Service Coordination

    • Assure implementation of program goals and contracting requirements in alignment with recovery-based services and quality indicators.
    • Develop and maintain strong protocols to ensure successful transitions of consumers to resources. Collaborate with DESC staff from other programs to ensure continuity of care for consumers.
    • Assure regular communication with other agency departments regarding service delivery and achieving program outcomes.
    • Coordinate psychiatric consultations, clinical supervision, program meetings, and in-service training for program staff; participate in clinical reviews and case conferences.
    • Develop and maintain effective ties with funding sources and representatives; attend relevant City, County, and State meetings. Assist with budget development and management.
    • Establish cooperative relationships with human service organizations within the community, focusing on building relationships with BIPOC and sexual minority agencies.

    Hiring and Staffing Programs

    • Maintain strong knowledge of DESC personnel policies, procedures, and collective bargaining agreements; assure compliance.
    • Provide routine clinical and administrative supervision for supervisors and managers of HOST programs.
    • Conduct performance evaluations of supervisors/managers on a scheduled basis; maintain supporting documentation in confidential files with HR.
    • Oversee that HOST programs are adequately staffed, developing a team that reflects the individuals served by the program.
    • Coordinate with Human Resources to advertise and conduct hiring effectively and efficiently, aiming for a diverse staff reflective of the populations served.
    • Participate in screening, interviewing, and hiring processes as needed.
    • Assist the project manager in educating supervisors and program staff regarding agency policies, procedures, contract requirements, and compliance monitoring.

    Contract Compliance & Quality Assurance

    • Maintain familiarity with all relevant DESC, County, and State licensing, contract, regulatory, and procedural requirements associated with service delivery; assure compliance.
    • Collaborate with the QIM contracts manager to ensure appropriate execution of programs; develop corrective action plans as required by funders or State licensing departments.
    • Collaborate with data analytics managers to ensure reports capture the work accomplished by the HOST team; improve data availability for illustrating program outcomes.
    • Coordinate with Quality & Information Services staff in conducting quarterly quality assurance reviews of clinical charts; assist with quarterly and annual auditing of the program.
    • Oversee compliance with documentation requirements from relevant WACs, King County, PHP, and other funding sources; ensure completion of all required program reports.
