    Job Description:

    • Utilize the nursing process to provide high-quality direct and indirect patient-centered care.
    • Coordinate patient care by working collaboratively with the interdisciplinary team.
    • Communicate the patient’s clinical condition and respond quickly and accurately to changes in condition or response to treatment.
    • Create and maintain a safe, structured, and therapeutic environment with age-specific behavioral management practices.
    • Promote the profession of nursing and professional practice through collegial support and interactions.

    Requirements:

    • Must be a graduate of an accredited school of Nursing and have a current Registered Nurse license with the state of Connecticut.
    • Certification in Cardio Pulmonary Resuscitation (CPR) upon hire and maintain clinical competency during employment.
    • Training in PMT and de-escalation techniques, certification required within 6 months of hire.

    Competencies:

    • Effective communication skills in English, both verbal and written.
    • Basic computer skills.
    • Ability to take swift and efficient action, including patient restraint in emergencies.

    Essential Duties and Responsibilities:

    • Perform comprehensive assessments and reassessments of the patient condition and care needs, including psychosocial, physical, educational, and cultural aspects.
    • Develop and revise individualized treatment plans based on assessment and patient response to treatment.
    • Follow medication rights and reduce the potential for medication errors.
    • Optimize patient safety and promote positive patient outcomes.
    • Document the nursing process and responses to care in a timely manner.
    • Demonstrate flexibility, organization, and function under stressful situations.
    • Formulate and evaluate teaching plans based on identified learning needs.
    • Serve as a professional role model and mentor.

    Professional Development:

    • Pursue personal and professional growth and development.
    • Participate in Early Response codes and Dr. Strong codes, and attend PMT refresher trainings.

    Work Environment:

    • Maintain a safe and therapeutic environment.
