    Job Description:

    • Committed to the safe and effective use of medications through maximizing drug distribution and optimizing drug therapy outcomes
    • Educate patients, providers, and ourselves on best practices
    • Fully integrate pharmacists into the continuum of care
    • Report directly to a designated Pharmacy Supervisor based on the area of practice

    Responsibilities:

    • Review the patient's medication profile prior to verifying each new order, evaluating for indication, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
    • Ensure appropriateness of therapy by reviewing patient medication history, assessing demographics, recommending formulary alternatives, developing drug monitoring plans, interpreting laboratory data, and monitoring pharmacokinetics for targeted drugs
    • Communicate drug therapy problems directly with the nurse and/or prescriber, documenting all drug therapy interventions
    • Perform medication histories, medication reconciliation, and discharge counseling as required
    • Provide drug information to healthcare professionals and patients
    • Detect, monitor, document, and report adverse drug reactions and medication errors
    • Maintain good work relationships and handle all communications in a professional manner
    • Supervise Pharmacy Technicians in daily work activities
    • Accurately dispense investigational drugs according to established protocols
    • Attend department education activities such as Grand Rounds/Noon Conferences, staff meetings, and development programs

    Requirements:

    • B.S. or PharmD in pharmacy
    • Licensed as a pharmacist by the NC Board of Pharmacy by the start date
    • Maintains an active NC pharmacy license and provides proof of renewal prior to December 31 annually
    • American Heart Association BLS is required within 30 days of hire

    Preferred Qualifications:

    • Hospital experience preferred

    Professional Development:

    • Obtain Continuing Education credits as required by the North Carolina Board of Pharmacy
    • Successfully complete all annual department competency assessments
    • Complete TJC annual requirements and occupational health screening
    • Follow hospital and departmental policy and procedures
    • Maintain confidentiality of medical records and comply with all applicable state and federal regulations, including HIPAA and HITECH
