Licensed Practical Nurse (LPN) Team Leader

Location: Lake Placid, NY

Job Description:

• Manage the daily clinical regimen of each resident and sub-acute patient
• Gain extensive clinical experience in a home-like setting
• Direct nursing care of residents, ensuring quality care and regulatory compliance

Responsibilities:

• Administer medications and treatments as ordered by the physician
• Direct and monitor personal care duties and nursing care procedures by Nursing Assistants
• Assist Nursing Assistants with direct care of residents
• Report any unusual symptoms, changes of condition, or needs of residents
• Follow and implement the Plan of Care for each resident
• Manage controlled substances and ensure proper medication storage and distribution
• Document in care plans, Nursing Notes, Medical Administration Records, and Treatment Records

Skills/Competencies:

• Exceptional customer service and problem-solving skills
• Strong decision-making ability
• Proficient computer skills
• Excellent verbal and written communication

Qualifications:

• Credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current state license
• Preferred training and work experience in a geriatric setting
• Ability to supervise and work well with other personnel
• Caring and respectful attitude towards residents
• Physical stamina for constant activity
• Ability to perform tasks to established standards of excellence
