Elderwood at Hamburg is searching for per diem and float Speech-Language Pathologists to add to our team. This position would be required to transfer to any Western New York Skilled Nursing Facility on an as-needed basis.
Job Description
- Assist with ensuring the health and well-being of individuals by making recommendations for a speech-language therapy treatment plan, if necessary, and carrying out treatments as planned.
- Conduct an audiometric screening of residents upon request and make recommendations for further audiology service.
- Screen residents for communication problems upon request; conduct an in-depth evaluation and make written recommendations for a therapy plan of care, if necessary and upon request.
- Coordinate audiometric screening tests for residents and make referrals for further audiology evaluations or treatments if necessary.
- Initiate speech-language therapy plans of care according to physician orders; maintain required contact with the physician during the therapy program of residents.
- Observe rights of residents and treat residents with respect and kindness.
- Maintain written evaluations, progress notes, and discharge summaries for all residents in a speech-language treatment program.
- Confer with other staff regarding special needs of residents and integrate speech-language treatment programs with goals and approaches of other disciplines.
- Attend Resident Care Planning and Family/Resident Care Plan Review meetings for residents currently in a speech-language treatment program.
- Maintain records that include statistical reports about residents, in-service education information, budget information, and a material/equipment inventory.
- Submit statistical reports to the Business Office staff for Medicare billing payments and complete assessment forms upon request.
- Attend staff meetings and required or preferred in-service training.
- Participate in developing and conducting in-service training programs for facility personnel involved in resident care.
- Review written speech-language policies and procedures annually and make necessary revisions.
- Maintain costs of the program within the parameters of the annual budget.
Requirements
- M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.
- State license to practice as a Speech-Language Pathologist required.
- Clinical Competence Certification from the American Speech and Hearing Association preferred.
- Ability to communicate well verbally and in writing required.
- Current, valid CPR/BLS credential.
- Qualities of maturity, diplomacy, and ability to work well with others required.
- Verification of qualifications by the State Department of Health Rehabilitation Therapies (license number/certification number) required.
- Ability to perform tasks to established standards of excellence required.
- Demonstrated willingness to uphold the vision and mission of the company by following the core values of integrity, teamwork, passion for excellence, and reverence for the individual is required.
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for all parties involved, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Compensation
- Speech-Language Pathologist per diem compensation: $45.00 / hour
- Speech-Language Pathologist full-time compensation: $33.00 - $44.86 / hour
