Licensed Clinic Director - Physical Therapist (PT) - Care Coordination Program

Job Description:

• Oversee how team members deliver care in accordance with federal, state, and local regulations.
• Support care coordination efforts by conducting wellness checkpoint visits, communicating with patients' providers and caregivers, and assisting with telehealth visits.
• Lead operational initiatives, including implementing company-wide policies, clinical programs, and quality initiatives.
• Advise interdisciplinary care partners on treatment planning and therapeutic interventions.
• Maintain professional practices and ethical standards in accordance with company policies and clinical initiatives.

Physical Therapist Responsibilities:

• Provide physical therapy and wellness services for older adults.
• Conduct assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team.
• Contribute to program development, quality improvement, and problem-solving.
• Optimize each patient's functional well-being and satisfaction.

Schedule:

• Flexible scheduling available. Clinics typically operate 8 a.m. to 5 p.m. Monday through Friday, allowing you to craft a schedule suited to your lifestyle.

Qualifications:

• Completion of an accredited Physical Therapy program (New Grads Welcome).
• Current state licensure in PT.
• Excellent verbal and written communication skills.

Compensation:

• $47.00 - $55.00 per hour
