Job Description

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
• Promote wellness and prevent illness and injury in various settings for individuals, families, and communities
• Work autonomously and in interdisciplinary teams as resources and consultants
• Positively contribute to safety outcomes and promote high-quality patient experience

Requirements

• Advanced Practice Licensure (APN) Licensure in the State of IL required
• Federal DEA and Illinois Controlled Substance License required
• Completion of Master's level program in specialty area
• BLS/CPR certification required
• Prior APN experience in palliative medicine/hospice preferred

Compensation Information

$57.69 - $75.77 hourly pay range. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
