    Community Access Program Coordinator

    Educate and assist families and patients about the Community Access Program (CAP), including appropriate use of services to foster access and reduce inappropriate use of emergency and hospital services, and prevent nursing home placement. As required, provide assistance with eligibility determination for behavioral health, substance abuse, and social programs. Will assist in identifying funding sources to qualify patients for community assistance from a variety of special funds and agencies. Coordinates and facilitates navigation and continuity of care via the Community Access Program (CAP) and primary medical home in conjunction with the development of a discharge plan of care from area hospitals and providers.

    This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of North Carolina Community Access Program. They are trained and assigned to coordinate care for patients and support patients, families, and caregivers in self-management, self-efficacy, and behavior change. They are also trained to manage the practice's patient population of the Community Access Program and participate in the practice's quality improvement process and performance evaluation.

    Requirements

    • Bachelor's Degree in Human Services or similar field
    • Self-starter with two years of experience as a Case Manager or similar role
    • Demonstrated skills working with a multidisciplinary clinical team
    • Strong communication skills and computer literacy
    • Hospital, outreach, or outpatient experience required
    • Knowledge of community resources

    Position Requirements

    • Must maintain a full, unrestricted Driver's License
    • Must have an automobile for travel and maintain current auto liability insurance
    • BLS certification must be completed prior to going to unit/department and maintained going forward

    Pay

    • $14.51 - $23.22 per hour
