    Job Description:

    • Conduct home visits to provide physical therapy services to patients according to a written physician's plan of care
    • Evaluate home environment
    • Assess muscle strength, mobility, gait, and range of motion with potential for rehabilitation
    • Direct physical therapy treatment
    • Instruct patients and families/caregivers in the use and care of therapeutic appliances
    • Determine priority needs for physical therapy
    • Report to physician on patient's reaction to treatment or changes in condition
    • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of the program
    • Train patients in the use of prosthetic devices
    • Identify patient and family/caregiver needs for other home health services and refer as necessary
    • Prepare and submit clinical and progress summaries based on the attainment of goals
    • Participate in discharge planning for patients
    • Provide physical therapy consultation to families/caregivers when indicated
    • Provide in-service education programs for nursing organization personnel as needed
    • Participate in peer consultation process
    • Supervise Physical Therapy Assistants according to organization policy and state regulations
    • Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the only skilled service

    Requirements:

    • Degree from a baccalaureate or master's program in physical therapy approved by an accredited organization
    • Licensed to practice as a physical therapist within the State, if applicable
    • Two years of appropriate experience as a physical therapist; community/home health experience preferred
    • Good verbal and written communication and organization skills
    • Current CPR certification
    • Licensed driver with an automobile that is insured in accordance with state or organization requirements and in good working order
