Home Care Physical Therapist
Location: Suffolk County, New York
Job Description:
- Conduct home visits to provide physical therapy services according to a physician's plan of care
- Evaluate the home environment for safety and accessibility
- Assess muscle strength, mobility, gait, and range of motion for rehabilitation potential
- Direct physical therapy treatments and instruct patients and families/caregivers on therapeutic appliances
- Determine priority needs for physical therapy
- Report patient's reaction to treatment or changes in condition to the physician
- Initiate physical therapy programs and instruct family/caregivers in phases of therapy
- Train patients in the use of prosthetic devices as needed
- Identify patient and family/caregiver needs for additional home health services and refer as necessary
- Prepare and submit clinical and progress summaries based on goal attainment
- Participate in discharge planning
- Provide physical therapy consultations to families/caregivers when indicated
- Deliver in-service education programs for nursing organization personnel as needed
- Participate in peer consultation processes
- Supervise Physical Therapy Assistants according to policy and state regulations
- Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the only skilled service
Requirements:
- Degree from a baccalaureate or master's program in physical therapy from an accredited organization
- Licensed to practice as a physical therapist in the state
- Two years of appropriate experience as a physical therapist; community/home health experience preferred
- Good verbal and written communication and organization skills
- Current CPR certification
- Licensed driver with an insured automobile in good working order
