Home Care Physical Therapist

Location: Suffolk County, New York

Job Description:

• Conduct home visits to provide physical therapy services according to a physician's plan of care
• Evaluate the home environment for safety and accessibility
• Assess muscle strength, mobility, gait, and range of motion for rehabilitation potential
• Direct physical therapy treatments and instruct patients and families/caregivers on therapeutic appliances
• Determine priority needs for physical therapy
• Report patient's reaction to treatment or changes in condition to the physician
• Initiate physical therapy programs and instruct family/caregivers in phases of therapy
• Train patients in the use of prosthetic devices as needed
• Identify patient and family/caregiver needs for additional home health services and refer as necessary
• Prepare and submit clinical and progress summaries based on goal attainment
• Participate in discharge planning
• Provide physical therapy consultations to families/caregivers when indicated
• Deliver in-service education programs for nursing organization personnel as needed
• Participate in peer consultation processes
• Supervise Physical Therapy Assistants according to policy and state regulations
• Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the only skilled service

Requirements:

• Degree from a baccalaureate or master's program in physical therapy from an accredited organization
• Licensed to practice as a physical therapist in the state
• Two years of appropriate experience as a physical therapist; community/home health experience preferred
• Good verbal and written communication and organization skills
• Current CPR certification
• Licensed driver with an insured automobile in good working order

Pay: $100,000.00 - $120,000.00 per year
