Job Description:
- Conduct home visits to provide services to patients with speech and language problems.
- Assess patient's speech and language abilities and deficits.
- Prepare initial assessments and perform periodic reassessments.
- Develop a plan of care in accordance with the authorized practitioner's plan of treatment and agency policies.
- Provide rehabilitative services for speech and language disorders to meet established treatment goals.
- Select and use appropriate diagnostic and therapeutic techniques and materials.
- Document type of treatment and patient's response on designated clinical progress notes.
- Complete case conferences, prepare discharge summaries, and complete documentation as required.
- Instruct health care personnel, patients, and caretakers in home therapy programs.
- Collaborate with other health care team members to ensure continuous coordination.
- Participate in inter-disciplinary and case conferences, staff meetings, and in-service programs.
Requirements:
- Master's degree and certification of clinical competence in speech pathology granted by the American Speech-Language and Hearing Association.
- License and current registration to practice as a Speech-Language Pathologist in New York State.
- Preferred: One year of supervised institutional experience and one year of home care experience.
- Meet state health status requirements.
