    Behavioral Health Clinician

    M Health Fairview has an immediate opening for a Behavioral Health Clinician to join the Solid Organ Transplant team based in Minneapolis, MN. This is a full-time, 80 hours every two weeks, benefit-eligible position located on the University of Minnesota Campus.     Job Description

    • Assist in the management of the overall health care needs of the enrolled patient population.
    • Conduct assessment and evaluation of behavioral health conditions by means of diagnostic and screening tools, clinical interviews, and consultation with the medical team.
    • Conduct diagnostic evaluation and document assessment in the medical record.
    • Confer diagnosis as appropriate.
    • Develop an individualized treatment plan of care.
    • Collaborate with patient, family, and other colleagues to develop a treatment plan of care.
    • Develop an individualized plan based on age, social and cultural considerations, condition, and other relevant needs.
    • Maintain required, appropriate, and timely communication and correspondence with treatment authorization and payer sources.
    • Contribute to the development and implementation of treatment plans in collaboration with the patient and primary care team.
    • Provide consultation to the medical team aimed at the prevention, identification, treatment, and management of behavioral health/psychosocial issues and conditions.
    • Function as an effective team member by working in a collaborative approach with primary care providers.
    • Demonstrate basic knowledge of psychotropic medications.
    • Understand integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional, and psychological dimensions of health.
    • Employ a primary care approach in addressing the behavioral/psychosocial aspects of routine and complex medical conditions.
    • Provide primary behavioral health interventions for identified mental health and substance use conditions.
    • Deliver population-based health care by providing individual, group, and community-based services.
    • Apply evidence-based behavioral interventions for identified medical conditions.

    Requirements

    • Master's or doctoral level degree in social work or psychology.
    • Two years of experience post licensure.
    • Licensed Independent Clinical Social Worker (LICSW) or Licensed Psychologist (LP). Licensed Graduate in Social Work will be considered under certain circumstances.

    Preferred Qualifications

    • Five years of experience post licensure.
