    Child, Tween, and Adolescent Day Therapy Programs

    The Child, Tween, and Adolescent Day Therapy programs, located at UMMC-Riverside, are responsible for providing focused psychotherapy, skills, and case management to our patients admitted to day therapy (PHP and Day Treatment) with the goal of children and their families stabilizing using therapeutic intervention, so they do not need emergency room or inpatient mental health level of care.

    Job Description

    • Demonstrates clinical competence in the treatment planning process.
    • Facilitates treatment planning and oversees management of each case.
    • Coordinates/facilitates the development of treatment plans by collaborating with patients, families, and the interdisciplinary team.
    • Develops individualized plans based on age, social and cultural considerations, clinical diagnosis, level of functioning, and other relevant needs.
    • Implements, reassesses, and adjusts treatment plans as necessary.
    • Demonstrates clinical competence to assess individual patient needs.
    • Completes an interdisciplinary assessment tool with each patient.
    • Updates interdisciplinary assessment tools each week.
    • Demonstrates clinical competence in disposition planning.
    • Interfaces with government agencies such as court, ombudsmen, and county services.
    • Completes referral processes for the next level of care.
    • Communicates with other providers in the community.
    • Sets up intake appointments.
    • Completes needed paperwork to facilitate this process (e.g., applications, letters of recommendation).
    • Demonstrates clinical competency to partner with a multi-disciplinary team in the treatment planning process.
    • Assists in developing individualized plans based on age, social and cultural considerations, clinical diagnosis, level of functioning, and other relevant needs.
    • Communicates frequently and effectively with the interdisciplinary team to ensure continuity and coordination of care.
    • Ensures documentation is thorough, timely, legible, clinically appropriate, and meets regulatory requirements.
    • Demonstrates competence in developing clinically relevant transition/discharge plans.
    • Collaborates with patients and families to provide input to the interdisciplinary team in developing discharge plans.
    • Develops discharge plans based on age, social and cultural considerations, clinical diagnosis, level of functioning, and other relevant needs.
    • Documents thoroughly, timely, legibly, and clinically appropriately.
    • Demonstrates clinical competence in conducting group therapy, family therapy, and psycho-education groups.
    • Provides individual, couples, and family therapy.
    • Documents thoroughly, timely, legibly, and clinically appropriately.
    • Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines, and pathways for care delivery.
    • Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
    • Promotes a culture of evidence-based practice by participating in activities such as utilizing evidence-based policies in clinical practice, attending learning opportunities about evidence-based practice, and engaging in research council activities.

    Qualifications

    Required

    • Education: Master's Degree in Psychology, Social Work, or closely related field.

    • Experience: Completion of clinical education and internship as required by graduate program.

    • License/Certification/Registration: Be licensed or provisionally licensed by the MN Board of Social Work as LGSW or be working towards independent licensure as an LMFT or LPCC. BLS required.
