Job Description

• Provide occupational therapy services to patients, including assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care.
• Instruct patients, families, and caregivers in the use and care of therapeutic appliances and determine priority needs for occupational therapy.

Requirements

• Baccalaureate or master's program (preferred) in Occupational Therapy approved by an accredited organization.
• Current Illinois license as an Occupational Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.
• Home Care Home Base experience preferred.
