Job Description

• Provide physical therapy services to patients, including assessing and evaluating therapeutic, rehabilitative, and functional status.
• Participate in the development of the total plan of care.
• Assess for muscle strength, mobility, and gait.
• Instruct patients, families, and caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy.

Requirements

• Baccalaureate or master's program in physical therapy approved by an accredited organization.
• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.
• Home Care Home Base experience preferred.
