    Social Worker for Assertive Community Treatment (ACT) Program

    Location: Long Island, New York

    Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program, which provides services to adults who have been diagnosed with a severe and persistent mental illness. The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. The Social Worker will be given an agency vehicle to use for home visits with the clients. Travel is required.

    Job Description:

    • Educate clients and their families about managing severe mental illness and meeting individual goals.
    • Conduct daily home visits using the agency vehicle.
    • Create individual treatment plans based on clients' needs.
    • Complete client assessments, relapse prevention plans, and housing applications.
    • Develop individual goals to include stable plans for medication, housing, and employment.
    • Provide evidence-based practice treatment to clients with severe and persistent mental illness.
    • Connect clients to treatment facilities, including our clinics and other community resources.
    • Perform crisis intervention using the crisis phone after hours and on weekends, as needed.
    • Conduct group meetings with clients and their families to help rebuild trust and relationships.
    • Travel is required using the agency vehicle.
    • All other duties as assigned.

    Requirements:

    • Master's Degree required.
    • LMSW/LCSW/LMHC required.
    • 1 year of experience working with the severe and persistent mental illness (SPMI) population required.
    • Ability to work independently and as part of a multi-disciplinary team required.
    • Excellent interpersonal and communication skills, both verbal and written, required.
    • Proficient computer skills, including Microsoft Office and Electronic Health Records/Documentation, required.
    • Ability to multitask and prioritize in a fast-paced environment.
    • Valid and clean New York State Driver's License required.
