    Physical Therapist (PT)

The Home Health Physical Therapist is responsible for delivering personalized rehabilitation services to patients in their homes, focusing on improving long-term health and wellness. This role involves conducting comprehensive assessments and developing individualized treatment plans tailored to each patient's goals.

    Job Description

    • Conduct an initial evaluation/assessment and report findings to the physician/provider and Clinical Manager, and develop the plan of care promptly for patients
    • Perform physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests on patients in the home
    • Document initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and update the plan of care as necessary
    • Complete comprehensive assessments and OASIS data, as indicated, on a timely basis
    • Assist with orientation, supervision, and consultation services to Physical Therapy Assistants
    • Observe, record, and report to Health Care Provider the patient's treatment, response, and progress, and promptly report significant changes and obtain orders to revise the plan of care
    • Communicate with physician/provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
    • Provide education and support to patients and their families
    • Other duties as assigned

    Requirements

    • Master's degree in Physical Therapy
    • Active Physical Therapy license in good standing in the state of practice
    • At least one year of experience working as a Physical Therapist
    • Must have a vehicle, a valid current driver's license, and automobile insurance
    • Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner
    • Home Health and Home Care Home Base documentation experience
