Job Description

• Support the operations of the Care Management and Service Coordination program (Care Management)
• Complete required assessments using applicable forms
• Provide pre-enrollment education on behalf of third-party insurance programs
• Assume care management responsibilities as assigned
• This position requires fieldwork as well as in-office work

Requirements

• NYS RN licensure is required
• Experience in community health roles is desired (but not required)
• Requires travel to client homes (local) along with work from home
• Self-motivated with focused energy and commitment to meeting client needs
• Caring and able to appreciate the unique differences of others and their situations
• Team player with the ability to lean on the support of others
• Committed to the team and clients, treating colleagues like extended family

Benefits

• Salary: $58,000.00 - $70,000.00 per year
• Full-time schedule: Monday to Friday
