Job Description

• The Nurse Liaison is an LPN or RN who advocates and promotes the facility's services to hospitals, case workers, discharge planners, physician offices, and other clinical providers.
• The Nurse Liaison follows up on hospital referrals, evaluates residents for proper placement, explains the facility's services, and maintains and develops referral sources.

Requirements

• Valid Wisconsin RN or LPN license
• Two years experience in assisted living, senior housing, or senior healthcare
