Job Description

• Provide direct patient care, including specialized end-of-life care.
• Implement and follow comprehensive care plans, including accurate and timely documentation.
• Educate patients and families on self-care and treatment plans.
• Coordinate hospice services with an interdisciplinary team to ensure quality patient outcomes.
• Manage pain and symptoms, ensuring comfort and dignity for patients.
• Participate in professional growth and quality improvement activities.

Requirements

• Graduate of an accredited nursing program.
• Minimum 1 year of nursing experience (community health/hospice or med/surg preferred).
• Active LPN/LVN license in the state.
• CPR certified and up-to-date on licensure.
• Strong communication and organizational skills.
• Valid driver's license with a reliable and insured vehicle.
• Passion for working with terminally ill patients and understanding of hospice philosophy.
