- Monday to Friday schedule
- 2 days per week 8:30 AM - 5:00 PM, 2 days per week 8:30 AM - 7:30 PM, and Fridays 8:30 AM - 2:00 PM
- Active NY license and DEA required
- 5 primary health locations within Nassau County and southeast Queens
- 100% outpatient
