Good Shepherd, Inc., a senior living community in Mason City, Iowa, has an opening for a full-time Admission Coordinator, RN. This position is responsible for the collection, evaluation, and coordination of admission to Good Shepherd. The Admission Coordinator, RN monitors residents' health and well-being from the time of admission through the end of each resident's stay and updates care plans as needed. This position is responsible for assessing residents' mental cognition and physical function while maintaining and developing strong relationships. The Admission Coordinator, RN works closely with the nursing team, social workers, and MDS coordinators. Hours for this position are Monday - Friday, 8:00 am - 4:00 pm with some flexibility available.

Job Description

• Collect, evaluate, and coordinate admissions to Good Shepherd.
• Monitor residents' health and well-being throughout their stay.
• Update care plans as needed based on assessments.
• Assess residents' mental cognition and physical function.
• Maintain and develop strong relationships with residents and their families.
• Collaborate with nursing team, social workers, and MDS coordinators.

Requirements

• Current Registered Nurse (RN) license in the state of Iowa.
• Proven experience in case management or admissions within a healthcare setting is preferred.
• Strong understanding of medical terminology and healthcare regulations.
• Excellent communication skills to interact effectively with residents, families, and staff.
• Ability to assess patient needs accurately and adjust care plans as needed.
• Compassionate approach to patient care with a focus on enhancing quality of life for seniors.
