Job Description:

• Evaluate, plan, implement, and educate in conjunction with the client to achieve maximal functional outcomes for reintegration to home, work, community, and leisure activities.
• Provide quality rehabilitative services to persons including: referral interpretation, evaluation, treatment plans, implementation and modification, discharge planning, thorough documentation and assertive communicative skills with all persons involved in the care and support of the patient/client.
• Provide services for persons of all ages.
• Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering
