Clinical Social Worker (CSW)

Job Description:

• Facilitates foster care placement by providing services to children requiring foster placement and to their resource families.
• Conducts supervised visits, assessments, and develops individual case plans and treatment objectives for each client.
• Consults with the CSWS and Program Director to ensure quality and ethically sound services.
• Provides after-hours on-call assistance to families and programs on a rotating basis.

Key Performance Areas:

• Provides individual, group, and family counseling and supportive services.
• Makes regular home visits to approved resource families to assess and/or address the needs of children and to provide support to resource parents.
• Develops individual case plans and treatment objectives for children.
• Works closely with AGH Mental Health Services Department to coordinate all services and arrange appropriate referrals.
• Monitors that treatment objectives are met.
• Develops a comprehensive psychosocial assessment.
• Conducts assessments and screens as required, depending on the program.
• Monitors visits between children and birth families, providing intervention and support.
• Performs other duties as assigned.

Requirements:

• Master's Degree in Social Work, Counseling, or a related field recognized by Community Care Licensing.
• Maintain a welcoming environment for families and other community partners.
• Ensure compliance with HIPAA privacy practices.

Compensation:

• Salary Range: $66,560 - $82,000
• Starting Salary: $66,560
