Schedule: Monday, Wednesday & Thursday 8:30 a.m. to 8:00 p.m. Tuesday & Friday 8:30 a.m. to 5:00 p.m. Saturday 8:30 a.m. to 1:00 p.m.
EMR: Athena
Patient Age Demographic: Infant - Geriatric

Requirements:

Board Certification
IL state license
DEA
ACLS
Local preferred

Compensation and Benefits:

Hourly rates: $120/hr W2 and $140/hr 1099
Mileage reimbursement
