Outpatient Internal/Family Practice Physician

Job Description:

• We do NOT have a Pediatric population - High Geriatric Population.
• Occasional minor procedures: toenail removals, lesion removals, Pap Smears, Joint injections.
• Has excellent clinical skills.
• Is dedicated to delivering high quality, compassionate care to patients.
• Demonstrates a work history of positive collegial relations with colleagues, support staff, hospital-based providers, administrators, and patients.
• Demonstrates advanced competency in their specialty.

Requirements:

• Board Certified - REQUIRED
• Active AZ License - REQUIRED
• Fellowship - REQUIRED
• BLS, DEA - REQUIRED
• Availability at time of name clear - REQUIRED
• Malpractice - Clean Preferred
• Is licensed to practice medicine in the state of Arizona.
• Is board certified or board eligible in their specialty.

Schedule/Availability Requirements:

• Mon, Tue, Thur, Fri 7:30-5:30
• Setting: Outpatient Clinic
• Duties: Family Practice
• Days per month: 20
• Required Procedures: toenail removals, lesion removals, Pap Smears, Joint injections (negotiable)
• EMR: Cerner
• Support Staff: Medical Assistant/Triage RN/Front Office Staff/Clinic Manager
• Reason for Coverage: Provider retirement

Travel Preferences:

• Preferred Lodging: none. In the event that the Facility preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
• Air Travel: $750/round trip cap
• Rental Cars: $65/day all in cap (including taxes and fees) regardless of car class.
• Mileage: IRS Standard Rate
