Job Description:

Start: As soon as credentialled (End of September)
Schedule: 2 weeks per month, flexible for 4 to 5 days per week, 8a-4p (flexible on time).

Patients a day: 15
Act 10 is acceptable.
EMR: EPIC

Requirements:

Board Certification FM
WI license (Or IMLC)
DEA
ACLS, BLS
Clean Malpractice
Local candidates preferred
