Hospice Nurse
This position is offering a $25,000 signing bonus! As a member of the interdisciplinary team, the Hospice Nurse works under the general direction of the Director of Hospice and the attending physician in the provision of skilled nursing assessment, planning, and care to maximize the comfort and health of patients and families. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible for providing direct supervision of Home Health Aides and/or LPN's providing care for Guthrie Hospice.
Requirements:
- Registered Professional Nurse currently licensed in Pennsylvania
- Knowledge of nursing theory and practice at a level normally associated with an Associate's Degree
- One (1) year nursing experience in an acute care setting and/or previous Hospice experience preferred
- Experience and skill in supervision desired
Essential Functions:
- Assume primary responsibility for at least a one to fifteen patient/family caseload, including accessing, planning, implementing, and evaluating phases of the nursing process
- Initiate communication with attending physicians, other hospice staff members, and other agencies as needed
- Maintain regular communication with Hospice director, Medical Director, and attending physician concerning caseload
- Obtain data on physical, psychological, social, and spiritual factors and incorporate them into the plan of care
- Maintain up-to-date patient records
- Coordinate physical care of the patient by teaching caregivers or providing direct care as appropriate
- Meet regularly with hospice nursing staff and attend interdisciplinary team meetings
- Supervise and document hospice aide plan of care every 2 weeks
Other Duties:
- Travel is necessary for this position
- On Call is required
