Job Description:

• Provide skilled nursing services to patients in their homes to promote the patient's plan of care
• Assess, plan, implement, document, and evaluate care to patients and families
• Supplement the nursing care needs of the patient as provided by the RN
• Perform services in accordance with physicians' orders and in collaboration with Haven Home Health RNs, therapists, and other clinicians

Duties and Responsibilities:

• Recognize and report changes in the patient's condition to the RN supervisor
• Complete timely and accurate clinical notes including addressing patient's progress
• Assist the RN in carrying out the patient plan of care

Requirements:

• Current Missouri LPN license
• At least one (1) year recent LPN experience in acute, long-term, or home care preferred
• Home healthcare experience preferred

• Full-time

Pay:

• $54,600.00 - $72,800.00 per year
