Job Description

• Connecting with patients and their families
• Improving the quality of life of individuals with various needs
• Determining physical therapy treatment plans in consultation with physicians or by prescription
• Helping patients meet their goals and return to the highest standard of living as they are able
• Collaborating with our professional treatment team to create the safest, best situations for our patients, keeping them in their homes as long as possible
• Communicating client needs with their physicians
• Evaluating treatment plan, adjusting when necessary
• Documenting patient care services by completing necessary patient paperwork and department records
• Complying with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
