Job Description

• Provide physical therapy to home care patients and case management when indicated.
• Responsible for timely and accurate communication and documentation relative to physical therapy services.
• Promote and develop physical therapy services in homecare.
• Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
• Perform initial start of care assessment and all related forms.
• Develop, revise, and execute an appropriate care plan for restorative and maintenance patients.
• Collaborate with physicians, case managers, nurses, occupational therapists, physical therapy assistants, home health aides, and office staff to ensure consistent levels of care.
• Monitor patient progress and document accordingly.
• Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.

Requirements

• Bachelor's degree (preferred)
• Physical Therapy License (required)
• CPR Certification (preferred)
