Job Description

• Deliver healthcare services and treatment to elderly participants, ensuring their physical, mental, and social needs are met.
• Collaborate with a multidisciplinary team, including the Medical Director, Primary Care Physician, and PACE Director, to provide comprehensive care.
• Use your knowledge and expertise to assess, treat, and improve the health outcomes of frail elderly individuals.

Requirements

• Bilingual (English/Spanish) to communicate effectively with our diverse patient population.
• Must be a licensed Nurse Practitioner in the State of Massachusetts.
• Minimum one (1) year of recent, successful experience working with frail or elderly populations in a community-based or long-term care setting.
• Strong understanding of the physical, mental, and social needs of the elderly.

Job Details

• Location: Position requires travel to both Springfield and Newton. Must be able to commute to both locations.
• Hours: Full time
• Salary: $60-70/hour
