Job Description

• Maintain and actively promote effective communication with residents, patients, clients, and family members/personal representatives.
• Responsible for maintaining a positive image of the entity in the community, keeping in alignment with our mission, vision, and values.
• Attend training sessions, staff meetings, and in-services; complete required educational units annually and obtain a minimum of 12 hours of continuing education per year.
• Read and adhere to resident rights agreement and report any violations immediately.
• Treat residents, families, and other staff with dignity and respect.
• Use proper body mechanics in the performance of all tasks; maintain a safety focus.
• Only perform tasks in which they have been trained; will not operate equipment without proper instruction.
• Practice universal precautions and use personal protective equipment when dealing with blood and body fluids.
• Maintain a calm composed attitude when dealing with difficult residents, families, or staff.
• Perform personal cares and mobility of residents as assigned and as directed in the resident care plan.
• Maintain confidentiality.
• Maintain good attendance and take breaks only as assigned.
• Work cooperatively with all disciplines.
• Cooperate with scheduling clerk to accomplish adequate coverage for resident cares.
• Document appropriately in the medical record and effectively utilize electronic medical record.
• Report to team leader prior to the end of shift.
• Perform other similar duties as needed.

Requirements

• At least 16 years of age.
• Active license as a Certified Nursing Assistant with the State of Wisconsin.
