Impact Health PC is a Nurse Practitioner Group contracted with Skilled Nursing Facilities and Rehab Centers throughout the Northeast that strives to provide the best in medical support and consultation. We are hyper-focused on developing treatment plans that include the patient and their families in collaboration with facility staff, physicians, and specialists.

If you are an experienced Advanced Nurse Practitioner, know skilled nursing facilities, hospital emergency departments, SICU/ICU, long-term rehab, or have worked as a hospitalist, have experience diagnosing, treating, and prescribing, can stay calm under pressure, and thrive in busy environments that challenge your clinical skill set, this role is for you.

Our practitioners are vital members of the Medical Staff and considered primary care leaders, partnering with nurse managers, administration, and the medical directors to treat in place, successfully return short-term rehab patients home, and participate in facility initiatives that achieve optimal results.

Job Description

• Earning potential of $120,000 to $180,000
