    Home Health Nurse

    Join our dedicated team as a Home Health Nurse, where you will provide essential care to patients in their homes, ensuring their health and well-being through personalized treatment plans and compassionate support.

    Job Description

    • Develop and revise treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payors, including obtaining medical orders and approval of medical services.
    • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with agency-defined time frames.
    • Process orders and notify physicians of patient needs and changes in condition. Complete certification/recertification orders and discharge summaries.
    • Provide an environment that promotes respect for patients, their privacy, confidentiality, and property.
    • Initiate appropriate preventive and rehabilitative strategies.
    • Provide services requiring substantial specialized nursing skills.
    • Consult with and educate patients, families, and other team members regarding disease processes, self-care techniques, and preventive strategies.
    • Supervise LPNs to ensure patient health, safety, and compliance with the plan of care.
    • Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the Home Health Agency.
    • Interact with patients, physicians, referral sources, and others in a manner conducive to continued positive relationships.
    • Support Care Management philosophy through collaborative customer-focused planning and case management designed to meet individual health and service needs while promoting quality and cost-effective outcomes.
    • Ensure the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.
    • Maintain current knowledge of all available URMHC and community-based services.
    • Demonstrate flexibility and willingness to visit patients based on need rather than geography.
    • Ensure problem resolution at the point of contact.
    • Ensure authorization and release of information forms have been signed by patients or responsible parties.
    • Attend patient care conferences for patients assigned to your care.

    Requirements

    • Certified Home Health Care experience required.
    • EPIC or Netsmart experience required.
    • BLS certification.
    • ACLS certification.
