Internal Medicine Physician

Job Description

• Innovative, team-based model focused on improving health outcomes, enhancing independence, and reducing hospitalizations.
• Collaborative interdisciplinary approach supported by nurse case managers, medical assistants, pharmacists, and community resource advisors.
• Comprehensive, coordinated care including frequent patient visits, chronic condition management, and proactive patient education.
• Provide intensive, personalized care to medically complex patients.
• Educate and empower patients to manage their conditions effectively.
• Build lasting relationships and guide patients toward stable health and self-management.
• See up to 14 patients per day.
• Partner with experienced nurses and support staff for seamless care delivery.

Requirements

• Board-Certified or Board-Eligible in Internal Medicine.
• Hospitalist experience strongly preferred for managing complex, high-acuity cases.
• Passionate about chronic disease management and prevention.
• Strong communicator who values teamwork and long-term patient connections.
• Committed to proactive, holistic care.
