Home Health Nurse

We are seeking a compassionate and skilled home health nurse to provide personalized care to patients in their homes or assisted living facilities. The ideal candidate will have a strong nursing background and experience in various medical settings, including Level I Trauma Centers. This role requires assessing patient needs, administering medications, and collaborating with healthcare teams to ensure optimal patient outcomes.

Job Description

• Monitor and record vital signs to assess patient health status.
• Administer medications as prescribed and manage medication schedules.
• Provide vent/trach or g-button care as needed.
• Conduct diagnostic evaluations to determine patient conditions.
• Assist with discharge planning to ensure smooth transitions from hospital to home or assisted living.
• Utilize neonatal ventilators for patients requiring respiratory support.
• Perform suturing and wound care as necessary.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans.
• Educate patients and families about health management and treatment options.
• Stay informed about workers' compensation laws relevant to patient care.

Requirements

• Valid nursing license in the state of Texas.
• Experience in assisted living or home health settings preferred.
• Familiarity with vital signs monitoring, medication administration, and discharge planning processes.
• Knowledge of vent/trach and g-button protocols is a plus.
• Experience working in a Level I Trauma Center is advantageous.
• Strong communication skills and the ability to work collaboratively with healthcare professionals and families.
• Compassionate demeanor with a commitment to providing high-quality patient care.
