    Behavioral Health Navigator

    Assists members with behavioral health needs to navigate the health care system and access services available to them, under the guidance of a licensed clinical provider. Works as part of an interdisciplinary team to support the non-clinical needs of members with behavioral health needs.

    Job Description

    • Assists members with behavioral health needs to navigate services based on medical, social, and psychosocial needs, educates members on behavioral health programs under the guidance of a licensed clinical provider, and offers assistance as needed.
    • Assists members in resolving potential issues related to health care delivery, financial or social barriers, i.e., arranging for interpreters, transportation, etc.
    • Serves as point of contact for patients and families with behavioral health needs.
    • Assists with coordination of psychiatric and medical appointments.
    • Assists with coordination of services with outside agencies and other community resources, i.e., verifies appointments with members' case managers, assists with accessing outside programs, obtains pertinent medical records and discharge summaries, confirms outside referral authorizations for members.
    • Under direction from licensed clinical providers, assists members with referrals to intensive outpatient programs and partial hospitalization programs.
    • Assists patients with various form completion.
    • Assists with resource development. Networks with other community-based services.
    • Assists with departmental work lists to ensure appropriate follow-up and scheduling for patients.
    • Assists in planning and organizing groups and presentations in conjunction with other licensed providers.
    • Manages routine data collection and administrative duties in a timely manner, including hospital follow-up log, affiliated care providers lists, access reports, and other data collection reports as assigned.
    • Coordinates and participates in special departmental projects.
    • Works collaboratively with a multidisciplinary team through team meetings and oversight by licensed providers to ensure members are receiving quality care.
    • Attends and participates in scheduled staff meetings and training sessions.
    • Maintains required documentation using KPHC electronic health record.
    • Uses good judgment and discretion to support licensed clinical providers and appropriately documents actions taken.
    • Other duties as assigned.

    Requirements

    • Minimum two (2) years of experience in community outreach, health care, educational, or social service-related field.
    • Bachelor's degree in health and human services related field.
    • Basic Life Support required at hire from American Heart Association.

    Additional Requirements

    • Demonstrated knowledge of and skill in adaptability, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
    • Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications.

    Preferred Qualifications

    • Valid current Hawaii driver's license preferred.
