The Hospice Licensed Practical Nurse (LPN) works closely with RN Case Manager(s) to provide holistic patient-centered care and quality end-of-life care. As part of a collaborative interdisciplinary team, the Hospice LPN provides compassionate supportive care to our hospice patients, as well as their caregivers and families.

• One-on-one patient care visits in the home or care facility (ALF/LTC)
• Timely completion of required documentation, adhering to agency policy
• Attend and participate in weekly IDT meetings
• Manage comfort measures to ensure quality of life that is based upon the patient's goals
• Follow patient care plans and orders
• Coordinate with RN Case Manager, attending providers, and other IDT team members
• Engage in respectful communication (both verbal and written) with team members, patients, family members, and the community
• Adhere to agency-wide policies
• Other duties as assigned

Requirements

• Active NH LPN license
• Minimum 2-3 years direct care experience
• End-of-life or hospice experience preferred
• Excellent communication skills necessary
• Proof of flu vaccination
• Reliable transportation
• Valid driver's license
• Automobile insurance
• Physical exam within the last year
• Two-step TB test
